What identification (ID) do we need?
After you have completed and submitted the form you must bring 2 forms of identification (ID) to the surgery within 7 days.
One of these must contain your address (e.g. tenancy agreement, mortgage agreement, bank statement), and the other to have your photograph (e.g. driving licence, passport, for children a birth certificate is adequate).
Until you have submitted your ID you will not be registered at the practice.
