Our practice is part of Inclusive Health PCN. During 2025/26, the PCN and its member practices worked together to improve patient access, including telephone access, online consultation responsiveness, NHS App uptake, care navigation, continuity of care and patient engagement.
Our 2026/27 access plan builds on this work and focuses on maintaining multiple access routes, improving communication, supporting digital inclusion and strengthening continuity of care for patients with the greatest need.
